Introduction
Diabetes is a chronic disease that affects more than 100 million Americans (Center for Disease and Control and Prevention, 2019) and requires immediate attention. The purpose of this paper is to examine the compelling problem of diabetes and explore its significance and impact at the national, state, and local level. There are three common types of diabetes, but this paper will focus solely on type 2 diabetes. Addressing the chronic disease and management of diabetes will require the DNP scholar to propose interventions that directly impact disease process, evaluate clinical outcomes, and increase quality of care so that stakeholders can relate to the problem with identifiable solutions.

Implications of the Practice Problem at the National Level
Diabetes is one of the leading causes of death in the United States, comprising of 29.1 million American diagnosed with this disease (Russell, Vess, Durham and Johnson, 2017). The incidence of the diagnosis is approximately 1.5 million every year (Centers for Disease Control and Prevention [CDC], 2020). Diabetes is the primary cause of many health comorbidities and complications associating with more than 200,000 deaths yearly (Russell et al. , 2017). The economic ramification of diabetes is substantial, in which this disease consumes nearly 4.82% of the US health care budget (Murray, 2018). Diabetes is the most expensive disease to treat in America with an estimated health spending of $327 billion in 2017 (American Diabetes Association, 2018).

Impact of the Practice Problem at the Local Level on Key Stakeholders

Patients enter into healthcare in Pensacola, FL who are addicted to opioids. Local hospitals have criteria in which the patient must meet in order to receive a prescription for opioids. This places the client at an increased risk for inadequate pain control. In Florida, as of 2019, there are currently 22,923 children living in out-of-home-care (DCF, 2019). In the Northwest region of Florida, there are currently 2,446 children living outside the home (DCF, 2019). According to Shea and Graham (2018), 50% of all children removed from the home are the result of opioid abuse. Violence against nurses has increased, as local hospitals must have police in the emergency department.

Conclusion
Opioid addiction negatively affects the patient and their families, the economy, healthcare, childcare agencies, and healthcare workers at both the national and local levels. Nationally addiction is the second leading cause of accidental death (Peisch et al., 2018). At both the national and local levels, the foster homes are full, leaving workers scrambling to try to find a placement. The number of opioid- related deaths in FL aligns with the number of opioid- related deaths nationally (KFF, 2019). The doctorally prepared nurse has the ability to implement research into practice combatting this growing epidemic.

The Journey…. The DNP Practice Change Project: Tips for Success
Practice change projects, unlike research, focus on translating existing knowledge from research to practice to improve/influence outcomes. It is an interprofessional(IPC) approach to practice that bases decisions and practice strategies on transferring/translating best available evidence to influence a practice problem.
