Health information exchange (HIE) is the critical segment of health informatics through which structure different electronic record framework is shared and is possibly extraordinary for the health care system (Braunstein, 2014). It permits specialists, nurses, pharmacists, other health care providers and patients to appropriately access and safely share a patient’s fundamental clinical data electronically improving the speed, quality, wellbeing, and cost of patient consideration (Health IT, n.d.). The Exemplar HIE Governance Program permits the Office of the National Coordinator, ONC, to work cooperatively with substances previously associated with governance of health information exchange, empowering the proceeded with advancement and reception of approaches, interoperability prerequisites, and strategic policies that will expand the simplicity of electronic health information exchange, decrease usage costs, and guarantee the protection and security of information being traded (Nelson and Stagger, 2018). With the wealth of private data, there should have been a manner by which was ensured. The ONC made a top-notch network where patients and doctors can securely impart. Unexpectedly, suppliers can successfully share data. Understanding patient charts are saved coordinated in an effectively available area for accommodation which, thus, decreases clinical mistakes.

The HIE is essentially divided into three sets that include: Hybrid HIE, Centralized HIE and Federated HIE. Each plays a central role in the transmission of health information. Hybrid HIE’s store data centrally (Braunstein, 2014). This just indicates that the Hybrid HIE combines both the Central and Federated HIE’s, hence, the word Hybrid. Centralized HIE is the mastery repository used to store data (Braunstein, 2014). In this instance many hospitals usually use this as a method to either store or transmit confidential patient information. Lastly, the Federated HIE plays important role in the process of keeping patient information confidential. In the Federated approach the clinical data stays at one source (Braunstein, 2014). The Federated HIE is where all the data is basically brought together to exchange between other HIE’s. These systems are vital in the connection of coordinated care. Establishing and utilizing these systems has helped healthcare providers all over the world be able to successfully plan and care for the needs of their patients, which in some instance is vital to one’s health and survival.

In 2015 the state of North Carolina General Assembly passed a state-managed Health Information Exchange Authority also known as (NC HIEA). This was established to mandate and oversee health records of the State of North Carolina Health Information Exchange Network. The use of this system promotes the access, exchange and analysis of health information (North Carolina Department of Public Health, 2019). Taking a deeper look into my states HIE and comparing statistics with the Health IT site, I was able to see that a large portion of hospitals, medical groups, and providers here in the State of North Carolina have implemented the use of Electronic Health Records. Speaking from experience and working for one of the largest hospital systems in the Carolinas, I can honestly say that the use of EHR has shifted the way we manage a patient’s overall care. Electronic Health Records (EHR’s) are the first step to transformed healthcare. The benefits include improved health care, better health, improved efficiencies and lower healthcare cost and better clinical decision making (Saeed, 2018). Working in clinical case management I see first-hand the importance of EHR’s. With this incorporation we are better able to meet the needs of our patients and their care. If it’s from obtaining previous medical records/history or coordinating care with Skilled Nursing Facilities or Acute Rehabs the EHR is very important for communication and coordinating care. There is always room for growth and the HIE’s continue to improve daily.

Those of us who are called into healthcare have a required duty to protect the interest of our patients. God blesses us with the mind to create tools necessary to help others. Many lives have been saved and improved with the implementation of HIE’s all over the world. Health information exchange (HIE) initiatives and organizations provide an infrastructure to improve the required communication between public health and community partners (Nelson & Staggers, 2018). As healthcare employees we are all valuable assets in empowering and coordinating the care of the individual and the community in which we serve.

In the 1960s, Philip Morris decided to appeal to women through a new brand of cigarettes, the advertising slogan that dressed up smoking with an ad for Virginia Slim cigarettes, “You’ve Come a Long Way, Baby.” ( Advertising Age 1968a).  We could say the same for HIE, from paper charts and filing cabinets to electronic health records and health information technology, “You’ve Come a Long Way baby!” In response to the realities of an aging nation, ballooning health care costs, and calls for higher quality patient-centric care, agencies across the Department of Health and Human Services have committed to bringing new levels of quality, efficiency, safety, and patient-centric care to the U.S. health care system. Technology is widely acknowledged to be the primary vehicle for achieving these goals (Dullabh, Hovey 2013).

In 2010, ONC awarded $548 million to help states grow and promote assets to facilitate the exchange of health information among health care providers and hospitals within their jurisdictions, across the state and even support information exchange across to other states (ONC 2019).  In 2010, Texas Health and Human Services Commission received 28,810,208 dollars in award monies to help with efforts to build capacity for exchanging health information across the health care system both within and across states. Awardees are responsible for increasing connectivity and enabling patient-centric information flow to improve the quality and efficiency of care. The creation of a statewide health information exchange has allowed Texas healthcare providers to securely exchange health information among themselves. This has increased the coordination and quality of care while improving efficiency in the health care system and increasing consumer empowerment and control. (ONC 2019)

Texas created an e-Health Advisory Committee in July 2016 to advise the Texas Health and Human Services executive commissioner and HHS agencies on strategic planning, policy, rules, and services related to the use of health information technology, health information exchange systems. The tasks of the eHAC included: advising on the implementation, development, and long-term goals for health care information technology and Health Information Exchange (HIE), this included the use of computerized clinical support systems, electronic health records, HIE systems for the exchange of clinical and all other forms of health information. Another, very important task for this advisory board is to keep HHS agencies updated on incentives for increasing health care provider adoption and usage of an electronic health record and health information exchange systems. (hhs.texas.gov) Texas, like other states, has its work cut out for them but it looks like we are off to a good start.

Throughout my career in Texas as a respiratory therapist, I have had the opportunity to use electronic health records and health information technology systems by recording clinical patient information through a EMR vendor system. I must say it is wonderful to document in a patient’s chart in real-time, call the doctor that is on another floor or in the ED, and he can look at the same record and make changes, or look at test results, x-rays, and many other patient-related issues. I have seen first-hand how the electronic sharing of information is beneficial to the patient. In an emergent situation if a patient is being transported to another facility for a higher level of care, all of their health records, will be in the doctor’s hands before the patient ever leaves the ED.

The health information exchange made me think of how the written word of God has evolved and changed in the way it is delivered through the years. Scrolls, manuscripts, written and rewritten in many different versions and languages. Now in 2021, one can access the word of God in electronic form on their phone, tablet, or computer. With the advancement of technology, the gospel is unlimited and far-reaching to many that may have never heard the word before, especially in their own language. As Christians we must be as diligent about spreading the gospel as states are about using the HIE, the rewards are out of this world. Jesus gave us a command before he left, in Matthew 28:19-20 ESV “Go therefore and make disciples of all nations, baptizing them in the name of the Father and of the Son and of the Holy Spirit, teaching them to observe all that I have commanded you. And behold I am with you always, to the end of the age.”
