Providing educational resources and collaborating with different health care agencies such as stroke rehabilitation, cardiac rehabilitation, physical therapy and occupational therapy in providing continuity of care services and support for long term recovery after a stroke is crucial in regaining functionality. Educational resources will help stroke patient make healthy lifestyle choices to promote healthy living.
